Objectives and Resources

The Care Continuum is comprised of: The State Home Care Program, Frail Elder Waiver (FEW), Community Transition Liaison Program (CTLP), Money Follows the Person (MFP) Demo, Provider Network, Personal and Home Care Aide State Training (PHCAST), Behavioral Health Supports, and Clinical Assessment and Eligibility (CAE).
